We create fields in \"Settings\" first to keep naming consistent and prevent duplicate or mismatched fields later.\r\n\r\n### 01▸ Visit [app.gohighlevel.com](https://app.gohighlevel.com/) and sign in.\r\n\r\n### 02▸ Go to \"Settings\" → \"Custom Fields\" and click \"+ Add Field\".\r\n\r\n### 03▸ Choose the right field type (text, number, etc.). Click \"Next\".\r\n\r\n### 04▸ Give it a clear, descriptive name. Set up the field preferences relevant to the business requirements. Click \"Save\".\r\n\r\n### 05▸ Repeat for any additional contact custom fields needed.\r\n\r\n## Build the lead form\r\n\r\nIn this milestone, you will build a simple GoHighLevel lead capture form that collects standard contact details, adds any required custom fields, and prepares it for embedding on pages. Keep it streamlined using auto-mapped standard fields, with only necessary custom fields added.\r\n\r\n### 01▸ Go to \"Sites\" → \"Forms\" and click \"+ Add Form\".\r\n\r\n### 02▸ Select \"Start from Scratch\" and click \"Create\".\r\n\r\n### 03▸ Name it \"Lead Form – [Client/Offer]\" (Use a clear, standardized name so the form is easy to recognize later). Add \"First Name\", \"Last Name\", \"Email\", and \"Phone\".\r\n\r\n### 04▸ Drag and drop a custom field element, if necessary.\r\n\r\n### 05▸ Click \"Save\" in the form builder. You should now see the form in your \"Forms\" list with the name you assigned.\r\n\r\n\r\n## Create submission workflow\r\n\r\nThis milestone sets up an automation that fires whenever someone submits your lead form. Each submission is automatically tagged and placed into the correct pipeline stage, allowing you to skip manual updates.\r\n\r\n### 01▸ Go to \"Automations\" → \"Workflows\" and click \"+ Create Workflow\" and choose \"Start From Scratch\".\r\n\r\n### 02▸ Rename the workflow as \"Lead Form\" and add a trigger \"Form submitted\". Set filter to \"Form is\" = \"Lead Form – [Client/Offer]\". Click \"Save Trigger\".\r\n\r\n### 03▸ Add action \"Add Contact Tag\" and enter the agreed tag (create if missing under \"Tag\"). Click \"Save Action\".\r\n\r\n### 04▸ Add another action \"Create/Update Opportunity\" and set it to the existing \"Pipeline\" and \"Stage\" that you want and \"Status\" to \"Open\". Optionally set \"Opportunity Name\" to \"\\{\\{contact.first_name\\}\\} \\{\\{contact.last_name\\}\\} – Lead\". Click \"Save Action\".\r\n\r\n### 05▸ Click \"Save\", then toggle on \"Publish\" to activate the workflow. The workflow status should change to \"Published\". Newly submitted leads should now trigger the automation. Sometimes the first trigger may take a couple of minutes.\r\n\r\n\r\n## Embed the lead form on GoHighLevel page\r\n\r\nIn this milestone, you'll place your lead form onto a website page inside GoHighLevel. Embedding directly in HighLevel keeps all lead capture routing native and reduces setup errors compared to external embed options.\r\n\r\n### 01▸ If using a Funnel, go to \"Sites\" > \"Funnels\" and open the target funnel. Click \"Edit\".\r\n\r\n \r\n\r\n### 02▸ Now, click on the \"+\" button on your top left and select \"Forms And Surveys\". Under \"Add Existing\", drag and drop \"Add Existing Form\" element onto the page.\r\n\r\n### 03▸ Click on the existing form and select your form. The \"Lead Form – [Client/Offer]\" should appear by now. Once selected, the existing form will appear on the draft page.\r\n\r\n### 04▸ Optional - you can edit the form as needed.\r\n\r\n### 05▸ Click \"Published\" in the page editor, then set the correct domain and click \"Save\".\r\n\r\n## [Optional] Embed lead form on external site (get embed code)\r\n\r\nIf you need your GoHighLevel lead form on a non-HighLevel site (like WordPress, Wix, or Squarespace), you can embed it with a small code snippet. Leads will still flow into your HighLevel CRM.\r\n\r\n### 01▸ Go to \"Sites\" → \"Forms\" and open your lead form.\r\n\r\n### 02▸ Click \"Integrate\".\r\n\r\n### 03▸ Copy the embed code. You’ll see options for a script embed (matches site styling) or an iframe embed (more isolated, less flexible).\r\n\r\n### 04▸ Paste the code into the destination page’s HTML in your external CMS.\r\n\r\nFor WordPress, use an HTML block. For Wix, use the embed widget. For Squarespace, use a code block.\r\n\r\n### 05▸ Publish the external page.\r\n\r\nOpen the live page and confirm the form renders and accepts a test submission. Sometimes changes may take a minute to appear.\r\n\r\n## Run live test and verify CRM updates\r\n\r\n### 01▸ Open the live page with the form and submit a test entry.\r\n\r\nFill in all fields, including any custom ones, then click \"Submit\".\r\n\r\n### 02▸ Confirm the thank-you page loads.\r\n\r\nYou should see the thank-you page right after submission.\r\n\r\n### 03▸ Go to \"Contacts\" and open the test contact.\r\n\r\nYou should see the name, email, and phone number exactly as you entered them.\r\n\r\n### 04▸ Go to \"Opportunities\" and locate the new opportunity.\r\n\r\nYou see a new opportunity with status \"Open\" in the configured pipeline and stage. Sometimes this may take a couple of minutes to appear.\r\n\r\n### 05▸ Return to \"Contacts\" and open the test contact again.\r\n\r\nThe agreed tag, such as cold lead, should appear under \"Tags\" on the contact record.","html":"
SOP 286: GoHighLevel Lead Capture Form Setup
\n
This SOP covers building a native GoHighLevel lead form, mapping fields to CRM, tagging, adding to a pipeline stage via a workflow, and embedding on funnels/sites or externally.
\n
Last Updated / Reviewed: 11/13/2025
\n
Execution Time: 30–60 minutes (depending on pages and external embed)
\n
🚀 Goal: Build a simple GoHighLevel lead form, map fields to CRM, auto-tag, and place new leads into a pipeline stage.
\n
🤞 Ideal Outcome: Every submission creates/updates a contact with correct fields, applies the tag, and opens an opportunity in the right pipeline/stage.
\n
🤔 Why this is important: A standardized form + workflow reduces setup time and prevents routing errors for small-agency lead gen.
\n
🧐 Where this is done GoHighLevel sub-account.
\n
🗓️ When this is done: Before launching any landing pages or paid campaigns that require lead capture.
\n
👨 Who does this: Account admin with sub-account access.
\n
__⚠️Prerequisites or requirements: __You need admin access to the client’s GoHighLevel sub-account.
\n
You need edit access to the target Funnel/Website pages or external CMS (if embedding externally).
\n
Refer to the SOP 276: GoHighLevel CRM Foundations (web version) for instructions on field creation, tagging, and integration mapping.
\n
Click here to get the most up-to-date version of this SOP
\n
Create and confirm CRM custom fields (settings)
\n
In this step, you’ll set up or confirm any contact custom fields in GoHighLevel so your lead form can accurately capture and map data into the CRM.
